  • Current MVRs for all drivers. MVRs must be dated within 90 days of the coverage effective date.
  • If fleet is comprised of 11 power units or more, please include the following:
    • Current Driver Schedule in Excel format.
    • Current Vehicle Schedule in Excel format. Please ensure the Year, Make, Model, Complete VIN, and current Stated Amount for each unit are included.
  • Most recent annual IFTA Report.
